After January 15, all registered villagers receive a Welcome Packet in the mail which
includes the following forms and information. If you lose your forms or need additional optional
health forms, you may download them here. You may also request forms by
calling 1-800-222-4750 or e-mailing clv@cord.edu.
Forms must be printed and mailed to the appropriate location.
IMPORTANT: Please be sure to include the session code
and your Village Passport number on all forms. You will receive a Village Passport number with
your registration packet.
